4.You have not, since your last issuance of a valid prescription pursuant to a physical eye examination conducted by a registered eye care professional licensed in your country, state, province or other applicable jurisdiction, suffered from any eye condition requiring treatment, and have not been advised to stop wearing contact lenses by an eye care professional or medical practitioner;
5.You understand that contact lenses must be properly cleaned, properly maintained and used in strict conformance with the manufacturer's instructions and under the direction of your eye care professional. Before using the contact lenses, carefully review all instructions and information provided to you by your eye care professional and/or by the contact lens manufacturer. Failure to follow instructions for the cleaning, care and use of your lenses could result in serious illness, disease, and possibly loss of vision.
